Step One

Visit the National Disability Insurance Scheme (NDIS) website and complete the Access Checklist to see if you are eligible or you can call them on 1800 800 110.

If you are over 65, you may be eligible for My Aged Care funding support, so please contact our Gateway team to discuss your options.

Step 2

Once you know you meet the basic eligibility criteria through completing the relevant checklist, call the NDIS on 1800 800 110 and request to complete the Access Request Form over the phone.

Ensure you gather documents and reports from your GP, specialists and allied health professionals on how your disability impacts your day-to-day function, as you’ll need to provide this information to the NDIS.

Step 3

Follow the instructions from the National NDIS to get started with your plan. This is the moment where you can let your service providers know that you have an upcoming planning meeting so they can help you prepare for this. When you meet with your Planner, you can discuss your service needs.
